Category I and Category III Coding Change Request form

The CPT^® coding change request form plays a vital role in maintaining and increasing the efficiency of the CPT process. It can be used to submit a coding change request for any one of the three categories of CPT codes: Category I CPT codes and Category III CPT codes–Emerging Technology with the exception of Pathology and Laboratory.  A separate coding change request form must be completed for this section of the CPT book.
