The kit is designed as a self-study CME activity for physicians. Physicians may earn CME by watching the instructional video reading the manual for clinicians (PDF, 1MB), completing the case discussions and answering the CME questionnaire. The case discussions, the CME questionnaire and the CME answer sheet are all provided at the back of the manual for clinicians. After recording your answers on the CME answer sheet, please return the form via mail or fax to:

A certificate documenting your participation in the CME activity will be forwarded to you upon the successful achievement of a score of 70%. Non-physicians may receive a certificate of participation for completing this activity.
